Enrollment Forms

Below are fill-in enabled forms to enroll your child in the Early Childhood University. All forms need to be completed, printed, and signed prior to your child's enrollment. The completed forms can also be saved for future reference.

If you have questions, please call ECU at 506-6900, or simply drop the completed forms off at the ECU office.
